What is Waiver of Premium Benefit?

If an Employee, under age 60, becomes Totally Disabled for six months without interruption, Voluntary Term Life coverage may be continued at no cost to the Employee.

A claim form must be submitted and if approved, Voluntary Term Life coverage will continue until the Employee is no longer Totally Disabled or reaches age 65, whichever occurs first.

How do I file a claim for this benefit?

You can download a form and instructions by clicking here or by contacting your Campus Benefits Office. Your claim for benefits must be received by Fort Dearborn Life Insurance Company with 9 months following the date Active Service ended due to your Total Disability.

Are there any other steps that I am required to do once I am approved for Waiver of Premium?

You are responsible for providing continued proof of Total Disability upon written request by Fort Dearborn Life Insurance Company.

How long does it take to process a Waiver of Premium Claim?

Your claim will be processed within 10 days following receipt of required proof and/or medical documentation supporting Total Disability as defined by the policy.
