What is EOI?

Evidence of Insurability is an application process in which an applicant’s past and present health information is provided in order to determine eligibility for certain insurance coverages. Evidence of Insurability information must be reviewed and coverage approved by FDL before such coverage becomes effective.

Why is EOI used?

Evidence of Insurability is utilized to protect an employer’s group insurance program against adverse risks and reduce the likelihood of disproportionate claims risks. This helps to stabilize the employer’s utilization and control the cost of their insurance program.

When is EOI required?

EOI may be required to enroll in Employee Voluntary Group Term Life or Spouse Voluntary Group Term Life or to increase/add other System insurance coverages. Your Campus HR/Benefits Office can help determine if EOI is required for the change being elected and the time frame in which you must complete the application.

Where can I obtain an EOI application?

The EOI application is available at www.fdl-life.com/ut. Click on the Forms link, scroll down and find the Evidence of Insurability (EOI) section and click on the EOI link. If you have trouble downloading an application from the website, it is also available from your Campus HR/Benefits Office.

What are underwriting guidelines?

Underwriting guidelines have been developed over the years that we have been in business and are designed to help adequately classify health, disability and life insurance risks. Care is taken to understand and comply with legislation and insurance department regulations and to treat all applicants fairly. The underwriting guidelines utilized by FDL are specific and proprietary to Fort Dearborn Life Insurance Company and are consistent with industry practices. They are reviewed on an ongoing basis to keep current with improved medical technology, social changes and industry trends. The underwriting guidelines are administered consistently and fairly in each case.

How are underwriting guidelines used?

FDL utilizes underwriting guidelines on all applications where evidence of insurability is required. Acceptance or denial of coverage is based on these underwriting guidelines.

What does initial period of eligibility mean?

A person is eligible to enroll in the System insurance plan within the first 31 days of employment. Those persons who decide to enroll in Voluntary or Dependent Life or increase some System insurance coverages after the initial period of eligibility will be required to provide evidence of insurability which is acceptable to FDL. Examples of when EOI is required are: 1) when adding or increasing Voluntary Group Term Life (VGTL) for the Employee and/or spouse following a qualified change in status event; 2) for new benefits-eligible Employees electing greater than 3 times annual compensation VGTL coverage amounts; 3) for Spouse VGTL coverage amounts of $15,000 or $40,000; and 4) for Retired Employees requesting to increase coverage; or for individuals returning to the System following a break from active employment status to obtain retirement status: $3,000 Basic GTL is guaranteed issue, EOI is required for the $7,000, $10,000, $25,000 or $50,000 amount.

What is the time frame for processing my EOI application?

Your EOI application will be active in the review process for 90 days. If no action is taken on your application due to non-receipt of requested underwriting information, medical records and/or exam/lab tests within 90 days from the date of the initial request for information by FDL, your file will be closed and you must reapply for the coverage you want. This may require that you have another Qualifying Life Event or wait for the next Summer Enrollment.

Who should I contact for underwriting questions or the status of my EOI application?

For underwriting questions or the status of your EOI application for life insurance
coverage(s), please call the FDL Medical Underwriting Department during regular business hours at (866) 628-2606 (option 2). Hours of operation are Monday – Friday, 8:00 a.m. - 4:30 p.m. CST.

Who is responsible for obtaining medical records and exam/lab tests, when requested and payment of any associated fees?

When medical records are requested, the applicant/patient is responsible for signing the authorization on the Attending Physician’s Statement form and providing it to the physician or medical facility listed on the form for completion. The applicant/patient is responsible for any fee charged for completion of the APS form/release of medical records.

When a paramedical examination, urinalysis, blood test, MD exam, EKG, etc. is requested by the FDL Medical Underwriters (based on the age of the person(s) applying for coverage and the coverage amount), the requested procedure(s) will be paid for by FDL. You will be notified in writing of the specific procedure(s) requested and the name of the paramedical facility that will be contacting you to schedule a convenient time/place to obtain the requirements.

Why did you return my EOI application to me?

Applications are returned for completion of missing information such as height/weight and questions that were not answered or explained. When your application is returned, you must complete the information requested in our letter and return the completed application to the FDL Medical Underwriting Department. Upon receipt, further determination of eligibility will be made for the coverage(s) applied for.

If I complete an EOI application during summer enrollment, when will my insurance coverage become effective?

If your application meets established underwriting criteria and is approved, your Campus HR/Benefits Office will be notified of our decision and coverage will be set up in accordance with the Group Term Life Policy.

What is the appeal process if I am denied coverage through EOI?

You may ask for reconsideration of the decision by submitting a request in writing along with any additional information to the FDL Medical Underwriting Department. The underwriting file and any additional information submitted for reconsideration will be reviewed by the Underwriting Manager. When the review has been completed, you will be notified in writing of the decision. A copy of the letter will be mailed to the contact person at your Campus HR/Benefits Office.
